WARNING Any person, knowing that he or she is not entitled to obtain information made confidential by the above law, who obtains or attempts to obtain such information, is guilty of a felony of the third degree.

A letter of request for your Police Accident Report may be made in writing if you are an involved party and not able to come into the department.

If your request is within the first 60 days of the date of the accident, you must include: a copy of your driver’s license to verify your identity and involvement in the accident, your case number, your notarized Sworn Statement, a check in the amount of $2.00 (payable to Temple Terrace Police Department) and a self-addressed, stamped envelope to the following address:
